CF Transition Meeting

You are invited to join the Cystic Fibrosis teams of Children's Minnesota and M Health for a virtual event on the transition process to adult care. This event will be held on April 12th, 6:00pm- 7:00pm central time.

This event is designed for CF patients, parents & caregivers in the age range of 18-21 years nearing transition to adult care at the M Health Cystic Fibrosis Center. Participants will hear from a panel of M Health CF providers and team members on the transition process, logistics and everything in between. There will be a chance for Q+A and discussion to help patients and families gain comfort and confidence as they transition their care to the adult center.
